1. Purpose
This policy outlines the standardized procedure for requesting, preparing, and releasing clinical reports at OPPAM. It ensures confidentiality, ethical compliance, and secure information handling in accordance with professional mental health standards.
2. Scope
- All clients receiving therapeutic services through OPPAM.
- All psychologists, clinical staff, support staff, and administrative coordinators involved in handling client information and documentation.
3. Policy Statement
OPPAM is committed to protecting client privacy and maintaining strict confidentiality. Clinical reports will be released only to the requesting client and only after verification of identity. No third party will receive client reports without the client’s explicit written consent.
4. Procedure for Requesting Clinical Reports
4.1 Submit Request
Clients must send an email request to:

Requests must be made from the same email ID used during registration.
4.2 Required Information (Compulsory)
The request email must include the following:
Subject: Request for Clinical Report – [Therapist Name]
Body:
Name of Client:
Name of Therapist:
Registered Mobile No & Email ID:
Purpose of Report: (Example: personal documentation / medical / educational / legal / other)
Preferred Mode of Receiving Report: (Email / WhatsApp – mention the exact number/mail) Requests
with missing details will not be processed until corrected.
5. Processing Timeline
- The therapist will prepare and release the report within 2 working days of receiving a complete request.
- If the therapist is on leave, in training, or otherwise unavailable, the client will be notified, and a revised timeline will be provided.
Note: OPPAM does not process urgent / same-day report requests unless medically mandated.
6. Privacy, Confidentiality & Verification
- Reports will be released only to the client’s verified email or phone number.
- Reports will not be shared with employers, partners, family members, legal representatives, or any third party without signed consent from the client
7. Ethical Considerations
OPPAM holds the right to:
- Decline requests that compromise client safety, privacy, clinical integrity, or legal compliance.
- Provide modified / clinically appropriate summaries when full disclosure may cause harm.
8. Disclaimer
Clinical reports are professional documents based on therapy engagement and clinical assessment.
They are not character certificates, moral certificates, or documents for personal disputes.
Clinical reports provided by OPPAM are strictly therapeutic documents intended to summarize the client’s psychological history, progress, or mental health condition based on clinical interactions and standardized professional procedures. These reports will not be modified, altered, or drafted to support personal disputes, influence legal proceedings, prove innocence/guilt, or justify actions in relationship, family, workplace, or court-related conflicts. OPPAM and its therapists do not engage in writing reports for the purpose of emotional manipulation, character defending, or defamation of any individual. Any request to tailor, edit, or reconstruct reports for persuasive, legal, or adversarial advantage will be declined to maintain ethical integrity and professional standards.
9. Policy Enforcement
Any breach of confidentiality, unauthorized release of reports, or falsification of identity will result in:
- Immediate suspension of report processing
- Possible legal action as per privacy and data protection laws
