What Is GAD Counselling?

Generalised Anxiety Disorder is not the same as being a worrier. It is a clinical condition in which worry is persistent, pervasive, and largely uncontrollable — covering multiple areas of life simultaneously, producing significant physical symptoms, and causing real, measurable impairment in daily functioning. For many Malayali adults, GAD has been present for years — often since adolescence — without ever being named as something that could be treated.

In the Malayalam-speaking community, GAD is particularly prone to misidentification — both by the person experiencing it and by those around them. Excessive worry in Kerala is often described as “over-thinking” (ഓവർ ചിന്ത), labelled as a personality characteristic rather than a treatable condition, and sometimes even framed positively as conscientiousness or responsibility. GAD is significantly associated with childhood environments characterised by criticism, high parental expectation, unpredictability, or overprotection. Most people with GAD have beliefs that worry is useful — “if I worry enough, I can prevent bad things” or “worrying shows I care.” These worry beliefs prevent the person from allowing uncertainty to be tolerated, which means the anxiety is never given the chance to naturally resolve. 2. GAD with Social Anxiety

Social anxiety involves intense fear of negative evaluation in social situations, while GAD involves worry across multiple life domains. When both coexist, the person worries both about specific social situations and about a wide range of non-social concerns. The social anxiety may be masked in Malayali clients by the cultural context of family and community — where strong social performance is expected and social anxiety is interpreted as shyness.

3. Acute Stress Disorder

Acute Stress Disorder is diagnosed when trauma symptoms — flashbacks, dissociation, avoidance, hyperarousal — arise within three days to one month after a traumatic event. It is the nervous system’s immediate attempt to process an overwhelming experience. Not everyone with Acute Stress Disorder goes on to develop PTSD — but early intervention significantly reduces that risk. Normal worry is proportionate, time-limited, typically focused on one identifiable current concern, and resolves when the situation resolves. GAD worry is excessive (out of proportion to the actual probability or impact of the feared outcome), persistent (more days than not for at least 6 months), pervasive (covering multiple areas of life simultaneously), difficult or impossible to control once started, and accompanied by physical symptoms like muscle tension, fatigue, and sleep disruption. The key distinguishing feature is control: most people can choose to stop worrying about something when the evidence does not support the concern. People with GAD find this extremely difficult — the worry continues regardless of reassurance or rational analysis.

The goal of CBT for GAD is not to eliminate worry — anxiety is a normal and adaptive human experience that cannot and should not be entirely removed. The goal is to change your relationship with worry: reducing its frequency and intensity, developing the ability to disengage from worry when it is not useful, building tolerance for uncertainty that does not require resolving through more worry, and eliminating the avoidance and reassurance-seeking behaviours that maintain the anxiety. Most clients completing CBT for GAD report that worry no longer runs their life — it becomes an occasional visitor rather than a permanent resident. The evidence for CBT achieving this is very strong.
